INDIVIDUALIZED EVALUATION FORM

At the end of each week, each student will receive a “personalized” skill evaluation form outlining the level of his current development and the individual areas that need improvement. We encourage each student to provide this information to his coach at the beginning of the Hockey Season and ask him to assist in working on the player’s skill development needs..
